The Smith County Health Department and the Smith County School System have partnered to provide seasonal influenza (flu) vaccinations at your child's school. The forms are located on the rmd.me/NxsXNWY1f5p website. Print the consent form, fill it out, and send it to your child's school no later than November 15. You do not need to complete a form if you do not wish for your child to participate.
